Blunt Traumatic Dissection of Right Coronary Artery Presenting with Acute Inferior Wall Infarction: Dilemma in Management

Indian Heart Journal — Volume 68, Supplement 2, September 2016, Pages S276–S279

A case report describing the diagnostic and management challenges posed by blunt traumatic dissection of the right coronary artery presenting as acute inferior wall myocardial infarction.

Overview

Blunt chest trauma is an uncommon but recognised cause of coronary artery injury, and dissection of the right coronary artery following such trauma can present diagnostically and therapeutically as an acute inferior wall myocardial infarction.
This report discusses the clinical presentation, angiographic findings and the considerations involved in managing traumatic coronary dissection, highlighting the diagnostic dilemma such cases pose in the setting of polytrauma.
The case underscores the importance of maintaining a high index of suspicion for coronary injury in trauma patients presenting with ECG changes and elevated cardiac markers, and the role of prompt angiographic evaluation in guiding management.
